So this is the coupon which at first glance looks sketchy - but I am not a store manager so I have NO IDEA if coupons often look like this.

But after a couple minutes thinking about it, it could easily make sense if manufacturer X makes an item that they label with the stores brands A, B and C. This should be common knowledge for a store manager.
Their website:

Looks like the coupon was made for a wide selection of brands and was separately printed or stamped for CVS at a later time.
Did they try the bar code?
Yeah, that looks awfully dodgy, I'm not going to blame them for questioning it. (I will blame whoever designed something like that, though!)
They scan it?
Here is the trouble here. Shit communication.
1) The coupon does look suspicious. How well did the cashier tried to get that across without being a complete dick? It can be a legit concern of their's. "I have never seen a coupon like this before. I've seen many coupons, but never one like this." Try scanning it... if it works... "well, how bout that." If it doesn't, say the machine doesn't recognize it. "Hey siri... First Quality Retail Services".
"OK, this is what I found." Oh... this is either a real good forgery, or this is actually legit. Even if it doesn't come up, at this point, you've shown a reasonable customer they've done the best you could do. That doesn't sound like this actually happened.
2) Cashier left his machine with a customer there. Who does that?
2a) Looking for a manager? Aren't there phones or communication devices at that store, making it unnecessary to leave the post?
3) Customer follows cashier. Umm... no... you
don't do that. Sure, the cashier is being rude as all heck, but you don't follow the staff. This isn't about making a point. Getting in people's faces isn't going to help your case.
4) Manager doesn't seem to do anything but assume the coupon is fraudulent. Christ, I remember being a cashier at Dicks Sporting Goods and getting cut off at the knees when a customer was wrong, I insisted, then contacted the manager, and short of just giving away the store, they'd make certain the customer got whatever they wanted even when not entitled to it.
5) Manager calls 911 because uppity black person isn't talking nice to him. Grow a fucking pair will ya?
